If the flange is too small, the nipple doesn’t fit entirely in the flange and can’t move and expand properly.
The shape of the nipple can also lend itself to the use of a flange that is too small. Nipples with a clearly defined base are less likely to be measured incorrectly, and therefore, less likely to be sized improperly. Some nipples are a bit trickier to measure as the base of the nipple is harder to identify, and measuring at the wrong point can result in flanges that are too small. In some cases, the base of the nipple is not actually the widest part and sizing is done based on a separate, wider part of the nipple.

The two sexes disagreed slightly on the ideal diameter of the nipple. The average areolae around 30–40 mm were favored by women. Men were more in favor of a smaller areolae of 30 mm. A small areola was found to symbolize youthfulness as it grows with maturation and pregnancies []. According to a study by Dixson et al. men’s preferences for women’s areolar size appear to be highly culturally specific. Dixson et al. stated the areolar pigmentation as an additional strong signal for maturation of the breast []. This feature was not included in this study.
In a mastopexy for small breasts, your plastic surgeon will make incisions around the nipple area, and potentially down the front of the breast, depending on the amount of lift needed. In most cases incisions along the breast crease are not necessary.
